Community Specialist Palliative Care Nursing Team

Our Specialist Palliative Care Nursing Team supports people in their home environment in the areas of Wolverhampton, South Staffordshire and the borders of Dudley, Sandwell and Shropshire. Referral may be made by your General Practitioner, Hospital Consultant or other Specialist Nurse.
Being told that you have advanced cancer or another life-limiting condition will give rise to many questions and concerns for you and your family.
Following initial assessment and discussion the Specialist Nurse will agree a supporting plan with you and your carers. We work alongside other health care professionals involved in your care such as your GP, Hospital Consultant, District Nurse and Social Worker etc. The aim is to ensure our shared skills are of maximum benefit to you and your family.

Your Specialist Nurse will advise on the management of troublesome symptoms, which may occur as a result of your illness and assist with a variety of problems you may encounter which may be physical, psychological or social. You may be offered the support of other Hospice services such as Day Care, Lymphoedema, translation services or Complementary Therapies if appropriate to your needs.
Your Specialist Nurse understands that you and your family may feel distressed and perhaps have anxieties associated with your illness. The nurse will give you the opportunity to talk about such feelings and endeavour to find ways of helping you cope with your situation. Our aim is to keep you comfortable and cared for in the place you wish to be, which, for most people is at home.
